Developmental Home Licensure

Are at least 21 years of age
Obtain a fingerprint clearance card issued by the Arizona Department of Public Safety
Pass the protective service registry checks
Are lawfully present in the U.S. and reside in Arizona
Are free of medical, physical, or mental health problems that could interfere with the provision of safe care for a child or vulnerable adult
Are able to provide a caring, nurturing, and positive home environment
Can provide a child or adult with a clean bed and sufficient bedroom space
Have sufficient income to meet the needs of your family
Are able to meet and maintain the life-safety requirements for the home or apartment

Respite Certification

Shall complete a written application
Shall be fingerprinted for a criminal history record check
Shall complete a Criminal History Self Disclosure Affidavit, and disclose any prior criminal history
Shall have a Central Registry Check completed by Safe Haven Services.
If care will be provided in the home of the applicant, an inspection of the home and grounds by Department of Health Services and Safe Haven Services shall be completed.
Be of reputable and honest character
The applicant shall provide Safe Haven Services with a minimum of (3) references that are familiar with the applicant and are not related to the applicant by blood or marriage.
Shall complete all pre-service training including CPR/First Aid & Article 9
