Developmental Home Licensure
- Are at least 21 years of age
- Obtain a fingerprint clearance card issued by the Arizona Department of Public Safety
- Pass the protective service registry checks
- Are lawfully present in the U.S. and reside in Arizona
- Are free of medical, physical, or mental health problems that could interfere with the provision of safe care for a child or vulnerable adult
- Are able to provide a caring, nurturing, and positive home environment
- Can provide a child or adult with a clean bed and sufficient bedroom space
- Have sufficient income to meet the needs of your family
- Are able to meet and maintain the life-safety requirements for the home or apartment
Respite Certification
Alternative Care Certification
- Shall complete a written application
- Shall be fingerprinted for a criminal history record check
- Shall complete a Criminal History Self Disclosure Affidavit, and disclose any prior criminal history
- Shall have a Central Registry Check completed by Safe Haven Services.
- If care will be provided in the home of the applicant, an inspection of the home and grounds by Department of Health Services and Safe Haven Services shall be completed.
- Be of reputable and honest character
- The applicant shall provide Safe Haven Services with a minimum of (3) references that are familiar with the applicant and are not related to the applicant by blood or marriage.
- Shall complete all pre-service training including CPR/First Aid & Article 9
